How is hospice care provided?

 

Hospice care is provided by loving caring hospice staff members.  Our intent is to give as much comfort as possible as the end-of-life nears.  Our hope is to help our patients live fully, completely and comfortably for as long as they are able.

How services are provided by each hospice team member
*   
The patient's physician can continue to direct
     care.
*    The Hospice Medical Director consults with
     physicians and provides guidance to the
     Hospice staff.
*    A registered nurse makes regular visits and evaluates the patient's
     condition, talks with the patient's physician, and teaches family or friends
     to be caregivers and to manage pain and other symptoms.
*    A nurse is available for emergency response 24 hours a day, every day. 
*    Home health aides provide personal care.
*    Social workers help resolve problems, counsel and arrange for community
     services.
*    A chaplain assists the patient and family with spiritual and emotional
     issues.
*    Our grief (bereavement) counselors help the family through the transition
     period following the patient's death.
*    Volunteers may provide companion care as needed.
